Sydänpuuta muodostuu kun puun runko ikääntyy ja läpimitta kasvaa. Sydänpuun muodostuminen lisää jonkin verran puuaineen tiheyttä, heikentää läpäisevyyttä ja vähentää puun sisältämää kosteutta. Heikon läpäisevyyden takia sydänpuun kyllästäminen on vaikeaa. Sydänpuu on kuollutta solukkoa, jossa elävän pintapuun varastoaineet on hajotettu ja käytetty.


Sydänpuu voi olla väriltään tummempaa kuin pintapuu ja niiden välillä voi olla myös kosteuseroja, kuten esimerkiksi männyllä ja lehtikuusella on. Tammella, koivulla ja lepällä taas ei ole kosteuseroa pinta- ja sydänpuun välillä. Uute- ja pihka-aineiden vuoksi sydänpuun tiheys voi olla suurempi kuin pintapuun.
